AGREEMENT TO PARTICIPATE IN THE NORTH SIMCOE GIRL’S HOCKEY ASSOCIATION

As parent/guardian of the undersigned player, I hereby give my permission for her to participate in the activities of the North Simcoe Girl’s Hockey Association.

I agree to abide by the Constitution and By-Laws of the Association and hereby waive all rights of action against the North Simcoe Girl’s Hockey Association, it’s Officers, Directors, Team Officials, Team Sponsors or their employees for injuries or damages sustained while engaging in or travelling to and from Association activities.

As parent/guardian of the undersigned player, I hereby give permission to the Association to release the above information to other members of the leagues for the purpose of supplying coaches and trainers with player information as well as for communication with other members.

As parent/guardian of the undersigned player, agree to follow the “Fair Play Code and Conduct”, as per Canadian Hockey Association.

I have read the Privacy Policy of the Association.

Occasionally there will be opportunities for photographs to be taken and published in the media. I give permission for my daughter’s photograph to be included.

The North Simcoe Girl’s Hockey Association offers an instalment plan to pay your registration fees. Family rates are available for families with two or more children. Players must be paid in Full by October 20th or they will be suspended until payment is received.

Note: Any returned cheques must be replaced by cash immediately, or your child will not be allowed to play.

Fees:

Fundamentals Registration = $300

Local League Registration = $400

Lower Lakes Rep Registration = $475

note: Midget A rep fee $550

a 3% surcharge is added to the registration fees to register online
